Property Transaction Form Instructions

  1. Complete building locations by indicating: building, wing, room, division and department of the current location and new location, if applicable. Use building numbers as assigned by the Space Management Office.
  2. Complete information regarding: inventory system, inventory number, description, code and manufacturer's serial number. All completed forms must be sent to the Inventory Office, 21 N. Park St. Suite 6101, phone 262-8738.
  3. Include any additional comments or instructions in the space provided.
